Uric Acid and Cholesterol: What Gout Patients Should Know
If you have gout, you may already know your uric acid number. But your cholesterol and triglyceride numbers can matter too. Many people only connect gout with joints and sudden pain, yet gout often travels with other metabolic concerns, including weight, blood pressure, blood sugar, and blood fats.
Here is the simple version: in a large US adult study, higher uric acid levels were linked with a less healthy lipid pattern. People with higher uric acid tended to have higher total cholesterol, LDL cholesterol, triglycerides, and apolipoprotein B, while HDL cholesterol tended to be lower.
That does not prove cholesterol causes gout, or that uric acid causes cholesterol problems. But it does suggest that gout care should look beyond the painful joint.
What is a lipid profile?
A lipid profile is a blood test that looks at fats in your blood. Most people know it as a cholesterol test.
Common parts of a lipid profile include:
- Total cholesterol, the overall cholesterol amount in the blood
- LDL cholesterol, often called “bad” cholesterol because high levels can add to artery risk
- HDL cholesterol, often called “good” cholesterol because it helps carry cholesterol away from blood vessels
- Triglycerides, a type of blood fat that often rises with excess calories, alcohol, sugar, insulin resistance, and some medical conditions
Some research also looks at apolipoproteins. Apolipoprotein B, often shortened to apoB, can reflect the number of cholesterol-carrying particles that may contribute to artery plaque. Apolipoprotein AI is linked with HDL particles.
Why this matters for gout
Gout happens when uric acid builds up and forms urate crystals in or around a joint. Those crystals can trigger the classic flare: sudden pain, swelling, heat, redness, and tenderness.
But high uric acid rarely appears in isolation. It often shows up alongside other metabolic issues. These can include abdominal weight gain, high blood pressure, type 2 diabetes, insulin resistance, fatty liver, and abnormal cholesterol or triglycerides.
For a person with gout, this means a flare may be the most painful part of the story, but it may not be the only health signal worth checking.
What the NHANES III study looked at
The research article used data from NHANES III, a large US health survey conducted from 1988 to 1994. NHANES studies are designed to represent the noninstitutionalized civilian US population.
The researchers focused on 14,130 adults aged 20 and older who had complete information. They compared serum uric acid levels with several lipid markers, including total cholesterol, LDL cholesterol, HDL cholesterol, triglycerides, apoB, lipoprotein(a), apoAI, and lipid ratios.
They divided uric acid levels into five groups, from lowest to highest, then adjusted the results for many possible confounders. These included age, sex, race, waist size, hypertension, diabetes, alcohol intake, smoking exposure, lipid-lowering medicine use, and dietary intake.
What the study found
After adjustment, the highest uric acid group still had a less favorable lipid pattern compared with the lowest uric acid group.
The study found that higher uric acid was associated with:
- Higher total cholesterol
- Higher triglycerides
- Higher LDL cholesterol
- Higher apolipoprotein B
- Lower HDL cholesterol
- Higher triglyceride-to-HDL cholesterol ratio
- Higher apolipoprotein B-to-apolipoprotein AI ratio
In the fully adjusted model, comparing the highest uric acid group with the lowest group, average differences included 0.29 mmol/L higher total cholesterol, 0.33 mmol/L higher triglycerides, 0.14 mmol/L higher LDL cholesterol, 0.09 g/L higher apoB, and 0.08 mmol/L lower HDL cholesterol.
The triglyceride-to-HDL cholesterol ratio also rose in a steady pattern as uric acid rose. That ratio is often discussed as a rough marker related to insulin resistance and metabolic health.
What this means in plain English
The study suggests that higher uric acid and unhealthy blood fat patterns often travel together. For gout patients, that is useful because gout may be one clue that the body’s metabolism needs a broader check.
Think of it this way: uric acid is one number, but it may sit in a cluster of other numbers. If uric acid is high, it may be a good time to ask about cholesterol, triglycerides, blood sugar, blood pressure, waist size, kidney function, and overall heart risk.
This does not mean every person with gout has high cholesterol. It also does not mean cholesterol medicine treats gout. The point is simpler: gout care is stronger when the whole metabolic picture is reviewed.
The study cannot prove cause and effect
This research was cross-sectional, meaning it looked at measurements from a point in time. That kind of study can show associations, but it cannot prove which problem caused the other.
There are several possible explanations:
- High uric acid may be linked with changes in fat metabolism.
- Insulin resistance may raise both uric acid and triglycerides.
- Diet, alcohol, weight, kidney function, and genetics may influence both.
- Uric acid may be a marker of broader metabolic strain rather than the main driver.
That caution matters. The practical takeaway is not to blame one number for everything. The takeaway is to use the pattern as a reason to check and manage risk factors early.
What to ask your doctor
If you have gout or repeated high uric acid, consider asking:
- What is my serum uric acid level?
- What is my full lipid profile, including triglycerides and HDL?
- Is my LDL cholesterol at a safe level for my heart risk?
- Do my results suggest insulin resistance or metabolic syndrome?
- Should I be checked for type 2 diabetes or fatty liver disease?
- How are my blood pressure and kidney function?
- Could any of my medicines affect uric acid or triglycerides?
- If I need gout medicine, how does that fit with my other health goals?
Simple steps that may support both uric acid and lipids
The basics may sound familiar, but they matter because they can support more than one number at once.
- Limit sugary drinks, especially regular soda and sweetened juices.
- Be careful with heavy alcohol use, especially beer, if gout flares are a problem.
- Build meals around vegetables, beans, whole grains, low-fat dairy if tolerated, and sensible protein portions.
- Choose unsaturated fats more often, such as olive oil, nuts, seeds, and fish when appropriate for your diet.
- Keep a steady hydration routine unless your clinician has told you to limit fluids.
- Move regularly in a joint-friendly way, especially between flares.
- Work with your clinician on weight, blood pressure, blood sugar, kidney function, and cholesterol goals.

When to get medical advice
Talk with a healthcare professional if you have frequent gout flares, very high uric acid, high triglycerides, high LDL cholesterol, low HDL cholesterol, diabetes, kidney disease, chest pain symptoms, or a strong family history of heart disease.
If you already take a statin, blood pressure medicine, diabetes medicine, or urate-lowering medicine, do not stop or change it without medical guidance. These medicines are often part of a larger prevention plan.
